This form is for use by community colleges only. Remember checks and money order payments will no longer be accepted unless the provider is a community college. All CE participants should be entered in using the Online CE Payment page.
Attention CE Providers! Online payments for CE classes are now accepted! By clicking: Online CE Payment you can submit both the required electronic attendance form and your payment. We will transition to this electronic system over the summer and July 31, 2007 is the final date for non-electronic payments. Both the ID and Pin are your four digit CE number. Effective April 1, 2006, the
Board will begin charging all CE providers $75 per class session.
January 10, 2006 the Board
approved the form allowing Continuing Education Providers to update CE class
sign-in sheets online. The form will become the mandatory method of CE
class attendance sheet submission April 1, 2006. Each provider is
responsible for inputting the correct class information as well as the name,
license number, license type and hours credited for each participant.
Providers must also maintain records of the sign-in sheets for four years as
required by 21 NCAC 14R .0103.
To input each class please follow the directions below:
Step One: Course Identification
Please click the link below and type in the name
of the Course Provider, the CE number, the course name, instructor and location
and credit hours. Please make sure the date and time showing are the
correct date and time for the class. Click Save Class Info and Continue.
